# Round 12 disposition

Reviewed frozen revision 18, SHA-256 `45d9fe6bc8363a1e92663c2ff2dba6dcc055482b663e64aa9baeaa7ddbdb9a69`.

- K3: complete full-plan report, no substantive findings.
- Opus: two substantive findings. Both corrected in revision 19.
- GLM: authority and history scopes completed with no substantive findings. Management stopped at its output limit with a partial report and no verdict. That pass is not counted as clean; smaller overlapping scopes will cover it.

## Opus 1: gateway counter recovery

An older local counter could prevent subsequent cleanup after restoration. Section 16 now reconciles an authenticated gateway head, advances the counter, and signs current trusted desired state. It never renumbers an old mapping. Revocation tombstones and recovery checks prevent restored backups from re-enabling revoked phones. Independent administrator recovery is available when trust cannot be established. This adds no step to normal approvals.

## Opus 2: token challenge sequencing

Section 16 now defines root-signed candidate creation, gateway delivery of the possession challenge, enrolled-phone response, and root-signed final activation. Candidate creation does not change the active mapping. Expiry, revocation and newer-token races cannot activate stale candidates. No biometric or approval notification is added.

## Review limits

This is a design review. Platform feasibility gates remain unverified. No user-facing approval policy has changed.
